How To Qualify For Help

Lifted Spirits provides Direct Financial Assistance to Hospitality professionals who are experiencing one of the following crises:

Injury

Illness

Death in the Family

Housing Disaster

A significant injury that has resulted in loss of work hours, income, and financial stability.

An illness or signficant sickness that has resulted in loss of work hours, income, and financial stability.

The loss of an immediate family member, such as a partner, child, parent, sibling, or caretaker.

A housing disaster such as flooding, fire, storm damage, or other crisis of household safety.

How The Process Works

The first step to applying for Direct Financial Assistance is to submit an Ask For Help form, which you can find at the top of our website or on the Ask For Help page. As you go through the Ask For Help form we ask for some basic information that helps to get us a jump start on processing your help request.

Please Note: The Ask For Help form is not an application. The form is required to start the application process.

Once your Ask For Help request is received, it will be reviewed by a Lifted Spirits case manager who will then reach out to you with further instructions. As long as you have clearly stated your crisis, and it fits within the four crises we are able to help with, we will then give you access to the full application. In the full application we will collect all of the information we require to verify your need and determine the appropriate amount of financial assistance for your situation.

The full application will ask for standard information across all four crisis categories, as well as additional documentation of your specific crisis, depending on the type.

The following are standard types of documents we require across all four crisis categories:

A current pay stub

Your lease agreement 

Your current utility bill(s): water, gas and electricity

In addition to the documents above, as part of the full application process we will also ask for additional financial information to verify your financial need. These documents are used to understand the financial impacts of the crisis you are experiencing,  and help us to make a fair, honest, and accurate assessment of your application. Your case manager will help you to resolve any questions or concerns you may have.

Crisis-Specific Documentation

We also require specific documentation for each type of crisis or incident you may be experiencing. The following are the types of documents we will need to see in order to understand the full impact of your crisis:

  • Medical verification (such as a signed doctor’s note), including diagnosis and expected time missed from work
  • Medical receipts, if applicable
  • Accident or injury report if possible (example: police report from car crash)
  • Medical verification (such as a signed doctor’s note), including diagnosis and expected time missed from work
  • Medical receipts, if applicable
  • Death certificate
  • Documentation verifying relationship to the deceased
  • Accident report, if applicable
  • Verification of funeral service (for example: funeral program)
  • Receipts from funeral expenses, including travel if applicable
  • Incident report from emergency service agency, such as fire department

Note: We take the security of your personal information very seriously. Lifted Spirits will never use your information for any purpose other than to verify your circumstances in order to determine your eligibility for Direct Financial Assistance. Your information will not be shared with anyone outside of Lifted Spirits unless we are required to do so by law. All information we receive from clients are stored securely and kept confidential. Any information we share with the public is fully de-identified and aggregated to ensure anonymity.
